- Weekly Visits for updates

- Call The Marketing Busters when you have a referral/discharge.
- Your facility’s assigned Clinical Outreach Specialist will come asses your patients within 24 hours.
- Your patients will receive options and solutions for a safe discharge rather returning home or into a community setting.
All patients will receive a case management visit to ensure care plans are being met. This also helps prevent re-admission.
